Job Description:
A company is looking for a Case Manager RN.
Key Responsibilities
Engages with patients and caregivers to develop individualized Care
Plans focused on disease management and health promotion
Monitors patient progress and addresses barriers to achieving Care
Plan goals
Coordinates care across the continuum and serves as the primary
contact for patients and clinicians
Qualifications & Requirements
Must hold a current, active RN License in Michigan; BSN
preferred
Case Management experience and CCM Certification preferred
3+ years of clinical practice in relevant healthcare settings
required
1+ years of Utilization Management experience desired
Experience in disease state management and population health
management is highly desired
